PEARLY PENILE PAPULES (Hirsuties Papillaris Penis)


Pearly penile papules (synonyms: PPP, P3, hirsuties papillaris penis, pearl necklace, string or perls, corona divina) are small 1-2 mm skin-to-white patches in a wreath around the base of the gloss penis. It is a physiological phenomenon that occurs in about 20-30% of men. It is seen more often in negroids and in uncircumcised men. There are a few to hundreds, neatly arranged regularly in a row at the base of the acorn. Histologically, pearly penile papules are benign angioplastomas. In the dermis there are many thin walled capillaries surrounded by fibroblast proliferation, sometimes also concentric fibrosis around the adnexes. The histology is similar to other acral angioplastomas such as adenoma sebaceum, subungual and periungual fibroids, fibrous papules or the nose (face), acquired acral angiofibroma, and oral fibromes.

Therapy:
No. Explain to the patient that this is normal, no genital disease, and that nothing has to be done.
Nevertheless, there are still patients who suffer from the presence of this condition and request removal. This is possible:
  • Cryotherapy with liquid nitrogen is successful in 80-90% of cases, provided expertly carried out, preferably with a closed cone, with the pellets being completely frozen 2 times. With the standard spray technique, this does not work well.
  • Electrocoagulation under local anesthesia. Gently pinch with 2 mm ball or cut with needle electrode.
  • Laser surgery with CO2 laser or fractional laser.
  • Surgical: dope and cut or shaved.
